Suppress period on invoices

In certain cases it is not desirable to show periods on charges on the invoice. There is not a setting on price list charge to control this.

Improved look of invoices with tiered pricing

The tiered pricing explanations at the bottom of the invoices are now longer shown in cases where there is only one tier or the price is volume-based.

Various bug fixes

• It was not possible to remove payment methods on accounts if there was only one payment method

• Fixed tax calculation for certain usage-based charges

• Fixed issue when creating ramp deals
